MNACHC is a nonprofit association of Federally Qualified Health Centers (FQHCs) and other safety-net providers located throughout Minnesota that provide comprehensive preventive and primary care services to all individuals, regardless of their ability to pay. Member clinics offer medical, dental, and mental health care to approximately 190,000 patients in urban, rural and tribal areas each year. The majority of patients served by these clinics are low income, uninsured, and medically underserved. www.mnachc.org

 

MDP LogoMinnesota Department of Health Diabetes Program
The MDP is dedicated to improving the health of all people in Minnesota by reducing the impact of diabetes. To achieve this, the MDP works to attain population-wide impact by collecting and publicizing state diabetes data to guide policy and program design, convening forums and facilitating effective stakeholder partnerships, translating health research into practice, and promoting innovative, effective, and culturally appropriate improvement strategies. www.health.state.mn.us/diabetes

NHCN is a management services organization for community clinics in the Twin Cities metropolitan area. It provides centralized business and administrative support to these member clinics. Community clinics provide high quality, accessible, affordable health care to primarily low-income and medically underserved communities.
