Conference Objectives
At completion of this conference, participants will be able to:

- Describe effects poverty has on the health status of a population;
- Identify effective, culturally-appropriate strategies for assessing, treating and managing diabetes in primary care settings;
- Identify resources in the community available to assist in prevention of diabetes and its complications;
- Plan chronic disease care that is sensitive to reducing health disparities in communities of color and in ethnic and low income groups;
- Discuss the effects of federal and state policy developments on the health care safety-net and its ability to provide care to underserved communities and under/uninsured people;
- Describe ideas for aligning clinic operations to support the Care Model; and
- Apply new financial tools to improve agency performance.
